Emblem Hospitals Procedures Conditions Kidney Disease-2025-26

One of the first kidney transplant programs in the world, the Montefiore Einstein Kidney Transplantation Program is an internationally recognized leader, providing lifesaving care to children and adults with end-stage kidney disease. For over 60 years, we have been at the forefront of kidney transplantation, having performed one of the world’s first pediatric kidney transplants and established the first dedicated pediatric transplant and dialysis programs in the nation. Our survival rates are among the highest in the nation.

One of the largest and most successful pancreas transplant programs in the nation, our outcomes are among the best in the country, with three-year survival rates exceeding national benchmarks. We deliver state-of-the-art lifesaving care to insulin-dependent individuals with advanced type 1 or type 2 diabetes, kidney failure and other complications. Since performing one of the world’s first pancreas transplants over 50 years ago, with the longest survival recorded at the time, we remain at the forefront of clinical care and research for pancreas transplantation and methods to prevent or treat organ rejection.

Why Choose Montefiore Einstein?

We’re ranked in the top 1% of all hospitals in the nation for complex specialty transplant care, including pediatric nephrology, according to U.S. News & World Report, and are an international referral site for the most complex pediatric and adult kidney transplant cases.

We’re also ranked in the top 1% of all hospitals in the nation for diabetes and endocrinology according to U.S. News & World Report, and are an international referral site for the most complex pancreas transplant cases.

World Renowned Multidisciplinary Expertise

Our team of transplant experts includes transplant nephrologists, urologists, medical and surgical oncologists, transplant immunologists, immunologists, transplant anesthesiologists, radiologists, pathologists, geneticists, virologists, transplant surgeons, diabetologists, endocrinologists, critical care specialists, psychiatrists, nutritionists, physical and occupational therapists and dedicated transplant coordinators, who collaborate to address your unique needs throughout the transplantation process. We’ll coordinate any additional care or specialized services you may need, bringing together experts from across our health system to ensure your care is fully integrated and comprehensive.

Increasing Organ Transplant Access (IOTA) Model

Our program is proud to take part in the Increasing Organ Transplant Access (IOTA) Model, an initiative created by the Centers for Medicare & Medicaid Services (CMS) to enhance access to life-saving kidney transplants for individuals living with end-stage renal disease (ESRD). This model focuses on removing barriers, promoting timely evaluation, and ensuring more patients have the opportunity to receive a transplant.

Kidney and Pancreas Transplant Evaluation & Postoperative Care

Equipped with the latest in imaging technology and therapeutic advances, our team of world-renowned physicians and nephrology specialists offers patients complete kidney and pancreas transplantation evaluation and postoperative care. To learn more about our surgical care, visit our main website page.

Our world-renowned specialists use advanced diagnostic imaging technology to evaluate potential living donors and recipients, including ultrasonography, computed tomography (CT), CT angiography, magnetic resonance imaging (MRI) and digital subtraction angiography (DSA), for preoperative and postoperative evaluation.

We are among the few hospitals in the United States to have a transplant histocompatibility laboratory that is a member of the Organ Procurement and Transplantation Network (OPTN). Our in-house American Society for Histocompatibility and Immunogenetics (ASHI)-accredited immunology lab provides state-of-the-art histocompatibility, immunology and immunogenetics testing, including human leukocyte antigen (HLA) typing and antibody and crossmatch testing. For patients needing dialysis access surgery, we provide in-house vascular access services, including venous catheters, arteriovenous (AV) fistulas and AV grafts.

To ensure successful transplantation, we optimize graft survival with advanced immunosuppressive therapies and novel immune tolerance protocols. In addition, we use specialized monitoring techniques to assess transplant rejection and other complications. These techniques include tissue biopsy, noninvasive and minimally invasive liquid biopsy and immunological tests to help optimize immunosuppressive therapy.

Our robust living donor Program offers a way for individuals to receive a life-saving transplant sooner. Family, friends and anonymous living donors participate in the Program, reducing the wait time for transplantation in matched recipients. Live donor transplant success rates have been shown to be superior to deceased donor transplant success rates.

For our patients in need of a kidney or liver transplant, we created the first-of-its-kind, revolutionary I Want to Live digital tool, which uses artificial intelligence (AI) and video to tell their story of why they want to live. This proprietary tool offers a novel and unique approach to helping those who need a transplant overcome the hurdle of asking for help and reaching potential donors.

The Montefiore Einstein Center for Transplantation provides a number of resources to educate and provide help and support for you and your family through the entire transplant process, including a dedicated transplant coordinator to help you navigate each step of your journey before, during and after surgery.

Our Helping Hands Program is a resource network of doctors, nurses, nutritionists, social workers and administrators who help people navigate and better manage their health and lives as they go through the transplantation process. It complements state-of-the-art medicine with holistic approaches to improve quality of life for patients, families and caregivers.

Kidney and Pancreas Transplant Conditions We Treat

Our transplant nephrologists do not manage chronic kidney disease in place of general nephrology care. Rather, they focus on transplant evaluation, candidacy determination, and post-transplant medical management in close partnership with referring providers.

Kidney Transplant Evaluation

  • Advanced chronic kidney disease (CKD stages 4–5)
  • End-stage kidney disease (ESKD)
  • Patients approaching dialysis or seeking pre-emptive transplantation
  • Patients on dialysis interested in kidney transplantation
  • Prior kidney transplant recipients with declining graft function
  • Patients requiring kidney re-transplantation

Pancreas Transplant Evaluation

  • Type 1 diabetes with kidney failure
  • Type 1 diabetes with severe hypoglycemia or hypoglycemia unawareness
  • Select patients with Type 2 diabetes, based on established clinical criteria
  • Candidates for:
    • Simultaneous pancreas-kidney (SPK) transplantation
