Our Services

The primary care practice at Montefiore Einstein Advanced Care provides continuity care to patients 18 and older. We deliver preventive care and chronic disease management through services that include annual physicals, routine health screenings and age-appropriate immunizations. We also diagnose and treat common and chronic health conditions such as hypertension, diabetes and arthritis, and we offer pre-operative check-ups and follow-up care after hospitalization and emergency care visits. The primary care practice plays a critical role in coordinating patient referrals and consultations with specialties as needed and provides on-site phlebotomy for our patients’ convenience.
